We offer state-of-the-art interventional treatments such as transcranial magnetic stimulation and electroconvulsive therapy. We partner with numerous primary care and specialty programs to develop multidisciplinary programs. We offer a wide range of types and levels of services for individuals suffering from depressive or anxiety disorders. We have intensive programs including partial or day hospital treatment programs, intensive outpatient programs and individual therapy.

Responsibilities

  • Conduct comprehensive psychiatric assessments, including mental status examinations and medical histories, to diagnose and develop individualized treatment plans.
  • Provide evidence-based psychiatric evaluation and medication management for patients with various mental health conditions, including mood disorders, anxiety disorders, psychosis, and substance use disorders.
  • Collaborate with a multidisciplinary team of healthcare professionals, including psychologists, nurses, and social workers, to ensure coordinated patient care.
  • Monitor and adjust treatment plans as needed to optimize patient outcomes, using a patient-centered and recovery-oriented approach.
  • Administer and manage psychotropic medications and therapies, ensuring compliance with regulations and best practices.
  • Maintain accurate and up-to-date patient records, progress notes, and treatment plans in compliance with hospital policies and legal regulations.
  • Participate in case conferences, team meetings, and treatment planning sessions to review patient progress and provide recommendations for care.
  • Educate and support patients and their families, promoting mental health awareness and providing resources for coping and self-management.
  • Stay informed about the latest advancements in psychiatric care through ongoing professional development and training.

Description

PAGNY has an opportunity for a per diem Family Medicine Physician - Health Equity for Adolescents in the Legal System (HEAL) with the Administration for Children's Services (ACS). PAGNY staffs several municipal hospitals and clinics within New York City through an affiliation agreement with NYC Health + Hospitals. PAGNY's corporate staff provides administrative, practice management, and human resources services in support of its more than 4,000 clinical providers.

The HEAL Program is a new NYC Health + Hospitals initiative to transition into being the primary provider of medical services to young people in city custody. The staff will be responsible for 24/7 medical care across two secure detention sites, two school-based health centers, and limited pharmacy services at seven non-secure sites (group homes). Services include comprehensive medical screenings, dental screenings and procedures, vaccinations and routine testing, responses to immediate need, and referrals for offsite care within and outside the NYC Health + Hospitals system.

Opportunity Details

  • Work locations = Crossroads Juvenile Center and/or Horizon Juvenile Center.
  • Coverage includes Day, Overnight, and Weekend.
  • Provide medical care to ACS adolescents while keeping safety and security measures paramount.
  • Take relevant patient history, identify patient needs, and make diagnoses.
  • Develop and implement treatment plans using diagnostic data.
  • Perform complete well-care examinations, including physical examinations.
  • Perform complete gynecological histories and physical examinations.
  • Write prescriptions and inform patients about medications prescribed; garner parent/guardian consents when needed.
  • Complete medical forms and documentation related to patient care.
  • Make and track all referrals to other agencies and providers, with follow-up care provided.
  • Assist with phlebotomy, as needed, to ensure smooth patient flow.
  • Administer immunizations, as needed.
  • Distribute, refill, discard, and discontinue medication for patients.
  • Complete Court Orders and all other ACS mandated paperwork.
  • Evaluate and treat ACS employees (ONLY on an emergent basis).
  • Provide health education on topics including, but not limited to, preventive medicine and counseling on chronic diseases, as well as parenting, prenatal, nutrition, family planning, medication usage, and stress management.

Qualifications

  • Current New York State Physician registration.
  • Board Certified in Family Medicine.
  • Minimum three years related experience in family, pediatric, adolescent urban populations.
  • Must pass all appropriate screenings to work in a secured setting with young people.
  • Spanish language proficiency preferred (Not Required).

Hourly Rate = $106.77 per hour.
