Job Summary

We are seeking a compassionate and dedicated 3rd Shift Direct Support Professional (DSP) to join our team. In this role, you will provide overnight support to individuals with Prader-Willi Syndrome by ensuring a safe, restful, and secure environment while remaining available to address any emergencies or resident needs that arise during the night. This position permits sleep during scheduled hours in accordance with the awake/sleep schedule outlined by the group home, while maintaining readiness to assist as needed. This is a meaningful opportunity to make a lasting impact and be part of a team that values person-centered care, collaboration, and growth both personally and professionally.

Key Responsibilities

  • Sleeping is permitted during the shift by following the awake/sleep schedule outlined by the group home.
  • Remain available in the event of emergencies or urgent needs (e.g., medication administration, medical concerns, hygiene assistance).
  • Ensure that the sleeping environment is safe and secure for all individuals.
  • Maintain a quiet and calm atmosphere conducive to sleep.
  • Assist with daily living activities such as hygiene, personal care, and meal support in accordance with individual protocols.
  • Support residents in following their Individual Service Plans (ISPs) and Therapeutic Behavior Plans (TBPs).
  • Foster independence and teach life skills through consistent structure, encouragement, and positive role-modeling.
  • Facilitate engaging, structured activities based on each resident's daily schedule and personal goals.
  • Encourage community integration and active participation in meaningful recreational experiences.
  • Document daily progress, behaviors, and medical concerns promptly and accurately using clear, professional communication.
  • Monitor residents' health, safety, and overall well-being with a focus on dignity and respect.
  • Administer medications following procedures and maintain accurate, up-to-date medication records.
  • Use only agency-approved behavior support techniques and report concerns or incidents as required.
  • Collaborate with Program Managers, Coordinators, and fellow DSPs to support residents' success and quality of life.
  • Attend team meetings and training sessions and contribute to ongoing, team-based treatment planning.
  • Communicate effectively and respectfully with residents, families, and coworkers, fostering trust and shared purpose.

Schedule

  • 3rd Shift starts between 8:00pm and 11:00pm with the shift ending between 6:00am and 9:00am (Monday thru Friday)
  • 12-hour Weekend Shifts starts at 8:00pm and ends at 8:00am (Saturday and Sunday required on a rotating basis.)
  • Sleep Period: Sleep is permitted between 9:00 PM and 4:00 AM.
  • Holiday shifts required as part of the rotation.
  • Flexibility required to meet the evolving needs of our residents.

General Description Of Position

The Contracting Coordinator performs important activities related to contracting with SCFHP's provider networks, in accordance with state and federal regulatory standards and guidelines, and SCFHP policies and procedures, and business requirements.

Essential Duties And Responsibilities

To perform this job successfully, an individual must be able to satisfactorily perform each essential duty listed below.

  1. Thoroughly understand the various contract templates and be able to customize the appropriate template based on the specific situation.
  2. Using SCFHP templates, draft, negotiate, and process contracts, amendments, termination notices and Letters of Agreement as requested, in accordance with state and federal requirements, and organized in a manner that adheres to SCFHP standards for management approval.
  3. Work cooperatively with the Manager, Contracting and Credentialing and the Director, Provider Network Management, and other members of the team to execute SCFHP contracting efforts.
  4. Perform all necessary tasks related to producing and delivering contract documents and monitor the status of these documents in the contracting process.
  5. Use contract management software to appropriately store contract documents and create a tickler system for managing contract status (i.e. effective, renewal and termination dates).
  6. Provide support to the contracting committee including, but not limited to organizing and scheduling committee and other meetings to support contracting activities, managing agendas, track actionable items, and communication with stakeholders.
  7. Assist the Provider Network Management Department in researching, developing, and drafting communications, resources and tools necessary to manage the SCFHP provider network. This may include regulatory guidance, updates to the provider manual, SCFHP website, SCFHP provider portal and ad hoc provider communications.
  8. Draft internal and external documents/correspondence related to contracting processes such as desktop resources, procedures, flow charts, audit tools, reports, and logs for management approval. Implement identified activities in order to demonstrate efforts to remain in compliance with state and federal requirements and SCFHP policies and procedures and business requirements.
  9. Execute internal activities related to onboarding or terminating providers, coordinating with other departments as necessary.
  10. Work in collaboration within the department and across other departments on ad hoc, regulatory, and special projects.
  11. Perform other duties as required or assigned.
