Job Description

Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices, standards, protocols and guidelines. Position Summary:
The Inpatient WOCN APRN is a highly skilled advanced practice nurse specializing in assessment, management, and prevention of complex wounds, ostomies, and continence-related conditions. This role provides expert consultation across inpatient units, delivering evidence-based care, promoting optimal patient outcomes, and supporting interdisciplinary collaboration. The WOCN APRN serves as a clinical leader, educator, and resources for patients, families, and healthcare staff.

Key Responsibilities:

Clinical Consultation & Patient Care:

• Perform comprehensive wound assessments, including identification of wound etiology and accurate staging/classification.
• Develop and implement individualized wound care plans, including advanced dressing recommendations and treatment modalities.
• Lead pressure injury prevention initiatives, including risk assessment, staff guidance, and protocol implementation.
• Provide ostomy care services, including preoperative stoma site marking and postoperative management.
• Deliver ostomy education to patients and caregivers, ensuring competence in self-care prior to discharge.
• Troubleshoot complex ostomy complications and provide ongoing support to optimize patient outcomes.

Collaboration & Interdisciplinary Care:

• Collaborate closely with hospitalists, surgical teams, nursing staff, and other healthcare professionals to coordinate comprehensive care plans.
• Participate in multidisciplinary rounds and case reviews to ensure continuity and quality of care.

Education & Leadership:

• Provide formal and informal education to nursing staff, providers, and ancillary personnel on wound care, pressure injury prevention, and ostomy management.
• Collaborate with Nursing Education to provide education to clinical staff and help maintain competencies associated with wound care
• Support onboarding and ongoing competency development related to WOCN practices.
• Contribute to patient and family education initiatives to improve health literacy and outcomes.

Nurse Practitioners and Physician Assistants Role Scope:

Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physicians.

Job Summary:

Oversee the clinical care coordination of patients undergoing electrophysiology (EP) procedures, such as catheter ablations, lead extractions, and other heart rhythm interventions.  Work closely with electrophysiologists, advanced practice providers, and other healthcare teams to ensure smooth, safe, and effective patient care.

·          Prior experience in electrophysiology or program coordination is preferred.

·          Demonstrated clinical assessment, planning, intervention, and evaluation skills.

·          Proficiency in electrophysiology procedures such as ablation, lead extraction, and device management.

·          Ability to interpret 12-lead ECGs, cardiac rhythms, and device data (pacemakers, defibrillators, heart monitors, smartwatch ECGs).

·          Experience with hemodynamic monitoring, central venous access devices, arterial lines, and IABP.

·          Strong patient/family education skills for cardiac procedures, medications, and follow-up care.

·          Must be able to effectively communicate information in English.  Ability to speak another language in addition to English a plus.

·          Membership in a professional nursing organization is preferred.

·          Strong Knowledge of high reliability.

·          Demonstrated strong decision making and problem-solving skills.

These issues include but are not limited to; respiratory failure, gastrointestinal bleeding, liver injury/failure, kidney injury/failure, sepsis, drug and alcohol withdrawal and a specialty in the care of the pulmonary hypertension population. Under the supervision of a Critical Care Pulmonologist, the unit is staffed 24 hours-a-day with a team of Physician Assistants and Advanced Practice Registered Nurses.

Job Summary:

Oversee the clinical care coordination of patients undergoing electrophysiology (EP) procedures, such as catheter ablations, lead extractions, and other heart rhythm interventions.  Work closely with electrophysiologists, advanced practice providers, and other healthcare teams to ensure smooth, safe, and effective patient care.

Responsibilities:

•                 Monitors departmental compliance with regulatory and compliance requirements.

•                 Provides both direct and indirect patient care appropriate to patients served to achieve desired outcomes for the patient with consideration of age and cultural differences.

•                 Assists management with operational activities of the department including development of nursing staff advancement based on contemporary safe evidence-based nursing practice.

•                 Identifies and resolves issues affecting the delivery of patient care. Performs quality assurance studies and assists with implementation of quality assurance and/or improvement initiatives.

•                 Serves as resource to staff and external contacts on issues related to patient care, patient flow and clinical standards, policies and procedures.

•                 Utilizes efficient decision making and problem solving to ensure quality service and promote patient and employee satisfaction.

•                 Communicate effectively with peers and interdisciplinary teams to achieve desired outcome for patients.

•                 Perform other related duties as required.

Qualifications

Education:

•                 Graduate of an accredited School of Nursing.   Associate’s degree in nursing.   Bachelor’s degree in nursing is highly preferred.

Licensure / Certification / Registration:

Required Credential(s):

·          Licensed Registered Nurse credentialed from the Connecticut Department of Public Health obtained prior to hire date or job transfer date.

·          Certification in area of specialty a plus.

One or more of the following:

·          Certified BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date.

·          Certified BLS Provider credentialed from the American Red Cross obtained prior to hire date or job transfer date.

·          ACLS (Advanced Cardiovascular Life Support) certification.

Qualifications:

·          Minimum 3 years of clinical nursing experience, with 1 year in a progressive leadership or supervisory role.

·          Strong clinical expertise in cardiac care, leadership in program coordination, and specialized knowledge of EP procedures and patient management.

·          Prior experience in electrophysiology or program coordination is preferred.

·          Demonstrated clinical assessment, planning, intervention, and evaluation skills.

·          Proficiency in electrophysiology procedures such as ablation, lead extraction, and device management.

·          Ability to interpret 12-lead ECGs, cardiac rhythms, and device data (pacemakers, defibrillators, heart monitors, smartwatch ECGs).

·          Experience with hemodynamic monitoring, central venous access devices, arterial lines, and IABP.

·          Strong patient/family education skills for cardiac procedures, medications, and follow-up care.

·          Must be able to effectively communicate information in English.  Ability to speak another language in addition to English a plus.

·          Membership in a professional nursing organization is preferred.

·          Strong Knowledge of high reliability.

·          Demonstrated strong decision making and problem-solving skills.

·          Excellent communication skills, both verbal and written. Use communication skills to foster a healthy work environment and facilitate positive patient outcomes and experience.

·          Demonstrates cultural sensitivity in all interactions with patients/families and co-workers.
